Mercy Health School-Based Clinic

Lorain Schools has partnered with Mercy Health to provide an onsite full-service health clinic at Washington Elementary School.  Students in any of our other 13 school buildings can visit the LCS Mercy Clinic via a free shuttle during the school day.  Parents and guardians do not need to accompany their student but they must fill out the necessary paperwork in order for their student to be seen by the LCS Mercy Clinic.   
 
Services provided are: 
  • Immunizations and flu shots for students
  • Physical exams
  • Vision and hearing screenings
  • Chronic disease management
  • Well child visits
  • Common concerns such as ear aches, bumps, bruises, etc
Mercy clinicians work closely with our School Health Professionals (Nurses) to ensure students are treated and can return to class with little time away from instruction. Mercy will work with health insurance to cover services, and will manage the proper paperwork with families who do not have insurance. Bilingual support is available to students and families who speak Spanish.

Parental Consent Forms

Must be completed prior to scheduling an appointment for a student who will be taking special transport.
